The Story
J is a remarkable person with a vibrant spirit who also happens to live with multiple developmental challenges, a learning disability, epilepsy, a chromosomal disorder, and autism. These conditions and unique differences have shaped his interactions and experiences from a young age, particularly after a significant increase in epileptic activity at the age of three.
His journey has been marked by intense efforts to find environments where his needs and aspirations could be met. J’s behaviours, which have included physical and verbal expressions of his frustrations, are understood as responses to overwhelming anxiety and unmet needs.
J had been moved several times to different care centres in hopes of helping him manage his communicative behaviours. Unfortunately, all these attempts did not result in any improvement. Following this, J received support from his family until they could no longer.
The Approach
Nurseline Community Services embraced J with a commitment to understanding and addressing the root causes of his distress. Drawing on the principles of Positive Behaviour Support (PBS), our team focused on comprehending the underlying triggers of J’s behaviours and crafting both affirming and effective interventions. This process began with a thorough assessment involving J directly, ensuring his voice was heard and integral to our planning.
Our approach emphasised stability, continuity, and the cultivation of trust through consistent support, enabling J to engage in preferred activities like football, golf, and fishing, which brought him joy and facilitated therapeutic interactions. The involvement of his family, especially his grandparents, with whom he shares a strong bond, remained a cornerstone of our strategy, ensuring that his social and emotional needs were continually nurtured.
The result
Initially, a planned transition faltered due to inconsistency, posing a risk to J’s well-being by disrupting the consistency of care crucial for his stability.
Our team intervened, covering shifts at a moment’s notice, thus ensuring J’s environment remained supportive and familiar. This led the multidisciplinary team (MDT) to re-evaluate and ultimately select Nurseline Community Services to extend its support whilst seeking a longer-term alternative.
After finding an alternative, both Nurseline Community Services and the care provider were committed to ensuring a seamless and beneficial transition for J, centred around his unique interests. A carefully planned, phased approach was adopted, facilitating regular and thorough exchanges of care plans and risk assessments between the teams.
These collaborative efforts ensured that J’s transition was smooth and anchored in a deep understanding of his needs and preferences, setting a solid foundation for his future care and support.
J. now resides in a supported living residency, which he regards as his forever home. He is a happier person with improved well-being and loves his favourite hobbies, like football.
This transition has been marked by significant improvements in his quality of life, with moments of joy and satisfaction becoming more frequent and meaningful.
Witnessing J’s journey—from moments of challenge to those of triumph—has been incredibly rewarding for everyone involved.
