We are looking for dedicated Healthcare Support workers to lead the delivery of person-centered mental health services directly in people’s homes, ensuring care is both effective and reflective of our community values.

Job Overview:

The Healthcare Support worker is responsible for delivering comprehensive healthcare services directly in people’s homes. This role is crucial in managing complex emotional needs, including behaviour that challenges and personality disorders, ensuring care is both effective and reflective of Unique Community Services’ values. You may be required to work a short (4–8 hour) or longer shifts. Shift patterns vary depending on the care package and support needs of each person we support.

Key Responsibilities:

  • Provide mobility support, including basic moving and handling.
  • Monitor vital signs such as blood pressure, temperature, and pulse.
  • Help with meal preparation, nutrition, hydration, and maintaining a clean environment.
  • Offer companionship, emotional support, and, when needed, financial assistance.
  • Administer medication and maintain accurate documentation and records.

Required Qualifications & Experience:

  • One year of previous experience in healthcare settings, in community or home-based care.
  • Experience in managing complex health needs, with a focus on mental health, autism, learning disabilities and working with diverse populations.
  • Driving license is mandatory for community work and transport of the people we support.
  • PMVA training is required.
  • Experience supporting people with complex health needs.
