Comprehensive Assessment

The CPNs (Community Psychiatric Nurses) will conduct a comprehensive assessment to collect detailed information.

Needs and Goals Identification

We will identify the needs and goals involving the person (and their family, if appropriate) who aspires to establish short-term and long-term objectives.

Person-Centred Care Plan Development

We will develop a person-centred care plan, setting timelines for review and evaluation.

Care Plan Implementation

We will implement the care plan to deliver the right care and support in place as outlined in the care plan.

Regular Review and Modification

We will regularly review progress with the person, conduct follow-up assessments, and modify the care plan as required to ensure positive outcomes.
