Too often, people with complex care needs are placed far from home, surrounded by unfamiliar routines, unfamiliar people, and no choice over happens next.  Hospital stays that could’ve been avoided. Discharges delayed because the right support isn’t available. Families exhausted. Professionals stretched. People left behind.

It doesn’t have to be this way.

At Catalyst Care Group, we believe care should start where people are – close to home, grounded in trust, and built around choice. People have the right to say yes and to say no.   To feel safe. To be heard. To receive care that feels like home.

We’re a family of organisations offering clinical, therapeutic and community-based support for autistic people, people with a learning disability and/or mental health needs.

We provide support that is immediate when it needs to be – through crisis response services that reach people in hours, not weeks. And we offer long-term care that is consistent, personal, and structured around each person’s life – not around systems.

Support is grounded in strength-based, trauma-informed practice and the PERMA model of well-being. We bring together multidisciplinary teams support to build practical, person -centred plans that work in real life.

Catalyst Care Group’s purpose is to transform healthcare in the UK by demonstrating humanised ways of working, creating a framework to shift perspectives, and fostering conversations on how we can support people out of long-stay institutional care and receive care in their own homes. Our approach supports local care and promotes the well-being of the people we serve with the goal of preventing the need for an acute hospital stay or long-term inpatient admissions.

We believe receiving care at home should always be the first port of call when meeting the emerging needs of people and families requiring support.

With one purpose in mind – to change the healthcare sector for the better, we aim to reduce the rates of delayed discharges and focus on providing care and support at home.
